Free COC test report templates (South Africa)
An electrical COC in South Africa is normally backed by a test report. The Electrical Conformance Board (ECB) publishes official templates for the readings (an electrical test report template for each type of test), and you can download each one free from the links below.
The ECB test report templates
- Additional test report (general)
Testing several distribution boards under one COC.
- Responsibility sign-off annexure
Designer, material specifier and installer sign off their part.
- Resistance at points of consumption
Resistance readings, 30 points per sheet.
- Earth and neutral loop impedance
Loop impedance readings, 29 extra readings per sheet.
- Insulation resistance
Insulation resistance readings, 30 per sheet.
- Polarity at points of consumption
Polarity check at each point, 30 per sheet.
- Number of circuits and points
Circuits and points counted for an initial COC.
- Continuity of ring circuits
Ring circuit continuity tests, 30 per sheet.

How to fill in a test report
- Download the template you need from the ECB page and print it, or fill it in on screen.
- Record every reading from your instruments on site, one row per circuit or point. Use a new sheet when the first is full.
- Sign it with the certificate, as the registered person who did the testing.
- Scan or photograph every page so all readings are legible.
Which annexures a job needs depends on the installation and the regulations. Confirm with the ECB or your registered electrician. See who can issue an electrical COC for the basics.
How to attach a test report in EZ COC
Open the COC record for the customer. You have two ways to attach the completed template:
- As the COC document. Upload the certificate and test report together as one PDF, or as a photo of the signed pages.
- As photos with a description. Add each page as a photo, pick a section such as Distribution Board, and write one line of description.
When you generate the PDF, your signed certificate comes first, untouched. The photographic record follows, one section per page, numbered through to the end with the COC number on every page. Later you can find the record by customer, address or COC number. Read more in how to combine a COC and test report into one PDF and how to build a photographic record of every job.
Frequently asked questions
Where do I get the official COC test report templates?
The templates are published by the Electrical Conformance Board of South Africa at ecb.org.za/coc/downloads. This page links to each one. The templates belong to the ECB.
How do I attach a test report to a COC in EZ COC?
Fill in the template, then either upload it as the COC document, or add it as photos with a section and a description. When you generate the PDF, your signed certificate comes first and the photographic record follows, page-numbered.
Is EZ COC affiliated with the ECB?
No. EZ COC links to the ECB templates and is not affiliated with the ECB. It does not issue or register certificates.
What is a COC test report?
A COC test report records the readings behind an electrical Certificate of Compliance, such as insulation resistance, polarity and earth loop impedance. The registered person signs the certificate, and the test report normally accompanies it. The ECB publishes annexure templates for the individual readings.
Do I need a test report with every electrical COC?
A 2016 News24 summary of the rules says the certificate is accompanied by a test report in the format approved by the Chief Inspector. Which annexures a job needs depends on the installation, so confirm with the ECB or your registered electrician rather than relying on this page.
Which readings do the ECB annexure templates cover?
The eight ECB templates linked here cover a general additional test report, responsibility sign-off, resistance at points of consumption, earth and neutral loop impedance, insulation resistance, polarity, number of circuits and points, and continuity of ring circuits.
Are the ECB test report templates free to download?
The ECB publishes the templates for download on its website and this page links to each one. We found no charge on the pages we link to. The templates belong to the ECB, and EZ COC is not affiliated with it. Check the ECB site for the current version before you print.
Keep the test report with the certificate.
File the certificate, readings and site photos in one record, then print one numbered PDF.
